This stretch jewelry is one of countless variations, you can also choose to stretch your ear with tapers / spears or claws. There is no practical difference, other than what you think looks best.
Since this piece of jewelry is made of steel, it is very heavy and can therefore act as a weight stretch. If you wear it for a long time at a time, you may find that it stretches larger than the size you are wearing.
Stretching is something that requires patience, and we recommend stretching as follows:
1mm (normal hole in the ear) -> 2mm
2mm -> 3mm
3mm -> 4mm
4mm -> 5mm (some may skip 5mm)
5mm -> 6mm
And 2mm at a time from here.
Stretch your ear slowly (over a week per size) to get the best possible result! We usually say that an ear can shrink back to a normal hole if you stay under 6mm. However, this is not a promise as there are many things to consider.
